At Zentro Care, we provide community nursing services that allow participants to receive hospital-level care in the comfort of their own homes, helping them maintain their health and independence.
Eligibility for Community Nursing Supports
Participants may access community nursing if:
• The support is required due to their disability.
• The support is not funded by another health system or service.
• The support is considered value for money.
• The need for support is ongoing.
Note: The NDIS does not cover short-term or temporary assistance at home following discharge from a hospital or rehabilitation facility.
Zentro Care offers a wide range of nursing services, including:
• Medication Management & Administration (including sourcing, pick-up, and intravenous administration)
• Health Assessments & Education (advice on home safety and equipment)
• Diabetes & Dementia Management
• Vital Signs Monitoring (blood pressure, pulse, temperature, and blood sugar levels)
•Training for participants, families, or support workers
• Mobility Assistance (e.g., transfers from bed to wheelchair)
• Personal Care (hoist assistance, dressing, showering, toileting, and grooming)
• Continence Assessment & Recommendations
• Wound Care
